General Questions

How does mobile physical and occupational therapy work?

Answer: Mobile PT/OT brings care directly to you—whether at home, work, or another convenient location. Instead of traveling to a clinic, you receive one-on-one, personalized treatment in your own space. This model allows for greater flexibility, convenience and customized care tailored to your needs.

Do I need a doctor’s referral?

Answer: In New York, you can see a physical therapist without a referral for the first 10 visits or 30 days under Direct Access. After that, Medicare and some insurance plans may require a physician’s order. If needed, Opt2Restore can coordinate with your physician to ensure a smooth start to your care.

What areas do you serve?

Answer: For aging-in-place consulting and home modification services, Opt2Restore serves Nassau County, Suffolk County and New York City. In-home outpatient rehabilitation services are currently provided within select Nassau County communities through Opt Physical Therapy Rehabilitation, PLLC.

Medicare & Insurance

Physical therapy services are provided through Opt Physical Therapy Rehabilitation, PLLC, a Medicare Part B provider. 

Opt2Restore LLC provides expert consulting, safety assessments, and aging-in-pace services that are private pay and not billed to Medicare. 

Medicare covers skilled physical and occupational therapy that is deemed medically necessary, such as post-surgical rehabilitation, mobility limitations or pain management.

Preventative care, wellness programs, and maintenance focused services may not be covered. In these situations, Opt2Restore offers private -pay wellness, safety and aging-in-place support to help individuals remain active, safe and independent at home.

You can book directly online:

  • Cash-Based Services: Schedule via HelloNote. 

  • Call us at (516) 860-0331 or (888) 973-5605 for assistance.

We accept credit/debit cards, HSA/FSA cards, PayPal, and Stripe payments for cash-based services. Medicare-covered sessions are billed directly to Medicare and secondary insurance.

Why would I pay cash if I have insurance?

Insurance-based physical and occupational therapy often includes limits on visit frequencies, session lengths, and covered services. 

Private-pay services through Opt2Restore provide: 

  • Longer, more personalized sessions (typically 60 minutes vs. shorter clinic visits)

  • Customized wellness, safety & performance programs beyond basic rehabilitation

  • Care guided by individual needs rather than insurance restrictions

These services are designed to support long-term function, independence, and quality of life.

Yes! Most Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) cover physical therapy. We provide detailed invoices for reimbursement upon request. Check with your provider to confirm eligibilit

Wellness & Preventative Care

Our Wellness Program is designed for individuals who want to stay active, prevent injuries and improve their mobility  beyond traditional rehab. Our wellness services focuses on long-term movement optimization, flexibility and strength building. This program is ideal for:

  • Seniors looking to maintain independence and avoid falls

  • Professionals with chronic pain or stiffness from prolonged sitting or repetitive movements

Wellness sessions are cash-based and customized to your functional goals.

Stretch Clinic

Our Stretch Clinic offers assisted stretching sessions to improve flexibility, posture and joint mobility. Sessions are geared to focus on deep muscle release and dynamic stretching techniques tailored to your body’s needs.

  • Helps with chronic tightness, poor posture, and mobility issues

  • Reduces muscle stiffness and enhances recovery

  • Ideal for individuals of all ages, from active adults to seniors

Sessions are available in-home or at designated locations. Cash-based pricing applies.

We begin with a personalized consultation to understand safety risks, mobility needs, medical history, and home environment concerns. From there, we create a clear, customized plan to support safety, recovery and long term independence.

  •  Lighting, flooring, and environmental hazards
  • Accessibility and adaptive equipment needs
