Comfort Uwadiae, Founder of Opt2Restore

Comfort Uwadiae, PT, DPT, CEAS, CAPS, ECHM

Founder & Principal Consultant

Dr. Comfort Uwadiae is the Founder and Managing Member of Opt2Restore LLC, a rehabilitation-led aging-in-place, care transition, and home modification consulting practice. She is also the Founder of Opt Physical Therapy Rehabilitation PLLC, a mobile in-home physical therapy practice.

Her rehabilitation journey began as a physical therapy aide at Professional Orthopedic Physical Therapy. Early clinical exposure, combined with growing up in a family of healthcare professionals spanning nursing, case management, social work, and physician care, helped shape a career dedicated to safe recovery, accessibility, and helping people live well at home.

With more than 14+ years of experience and nearly 30,000 patient and client encounters across acute critical care, inpatient rehabilitation, outpatient services, home-based care, and private practice, she brings a broad clinical perspective to recovery, accessibility, and healthcare-to-home transitions. A former Clinical Instructor and practicing provider at Montefiore Hospital, she maintains active clinical engagement through Catholic Health.

A home modification professional, rehabilitation-led aging-in-place specialist, and accessibility advocate, her work focuses on safe recovery, care transitions, accessibility, universal design, and practical solutions that support safety, independence, dignity, and confidence at home. She places emphasis on person-centered, thoughtfully integrated, and non-institutional solutions that help homes remain functional, comfortable, and aligned with everyday living whenever feasible.

She is the creator of the Opt2Restore™ C.A.R.E. Model, a framework designed to support safer care transitions, environmental risk reduction, and long-term independence. Her speaking experience includes local and national presentations on recovery, home safety, accessibility, universal design, aging-in-place, and healthcare-to-home transitions.

Grounded in Family, Community & Purpose

Beyond her professional work, Dr. Uwadiae values family, community engagement, and time in nature. She enjoys traveling, ice skating, and sharing her love of baking with family and community.

She is actively involved in the Hewlett-Woodmere school community through volunteer and leadership service, reflecting her commitment to family, service, and community connection.
