Do you accept Medicare?
Yes. Medicare-covered in-home physical therapy is provided by Opt Physical Therapy Rehabilitation, PLLC for eligible patients in Nassau County. Aging-in-place, home safety, home modification and care transition services are private pay.
Do I need a doctor’s referral to start?
In many cases you can begin with an evaluation. Some Medicare-covered services require a physician’s order, and we’ll help you understand exactly what’s needed for your situation before your first visit.
What areas do you serve?
Aging in place, home modifications and care transitions: all of Long Island (Nassau and Suffolk Counties), New York City, and parts of Westchester County. Virtual aging-in-place assessments and design reviews are available nationwide. In-home physical therapy: Nassau County, provided by Opt Physical Therapy Rehabilitation, PLLC.
What happens during the first visit?
A licensed physical therapist reviews your history and goals, then assesses your mobility, balance, strength, and home environment. Together you’ll build a personalized plan, all in the comfort of your own home, with no clinic travel required.
