What information should I include with a referral?

The essentials are the patient’s name and best contact number, the referring provider and practice, the primary concern or diagnosis, any recent hospitalization or discharge date, insurance type, and any known home-safety or mobility concerns. If something is missing, we will follow up with you directly.

Do you need a physician’s order before starting?

Requirements depend on the service and the payer. For therapy billed to Medicare Part B, a plan of care is certified by the patient’s physician. Assessments and consulting services may not require an order. Submit the referral and our team will confirm what is needed for that specific case.

How quickly will the referral be reviewed?

Referrals are reviewed within 1 business day. The patient or family is then contacted to discuss services and scheduling, and confirmation is provided back to the referring partner.

Which areas do you serve?

Opt2Restore serves Nassau County, Suffolk County, and New York City, including Manhattan, Brooklyn, Queens, and surrounding communities. If you are near the edge of a service area, contact us and we will confirm coverage.

Do you accept Medicare?

Yes. Opt2Restore accepts Medicare Part B and private pay for in-home physical therapy. Aging-in-place assessments, home modification consulting, and wellness programs may be private pay or potentially HSA/FSA eligible depending on the service.

Can I refer a patient who is still in the hospital or rehab facility?

Yes. Discharge-sensitive referrals are welcome, and earlier notice helps. Include the anticipated discharge date so we can coordinate the transition home and identify safety or mobility barriers before the patient arrives.

What is a C.A.R.E. Signal?

It is a secure form used to flag a high-risk, complex, or discharge-sensitive situation that may benefit from clinical and environmental review, before it becomes a full referral. Use it when you have identified a concern and want our team to take a look.

Will I hear back about my referral?

Yes. Referring partners receive confirmation once the referral is processed, along with clinical documentation aligned with your workflow where appropriate and permitted.
