This website is operated for two related New York businesses, referred to together in this policy as “Opt2Restore,” “we,” “us” or “our”:

• Opt Physical Therapy Rehabilitation, PLLC is a licensed New York State professional practice. It provides clinical physical therapy and is a Medicare Part B provider.
• Opt2Restore LLC provides non-clinical services: care transition planning, home safety assessments, aging-in-place consulting and home modification guidance. It does not provide medical care and does not bill insurance.

This policy explains what information we collect through this website, our forms, phone, email and text messages, and our services, and how we use and protect it.

2. Information We Collect

Information you give us

• Your name, phone number, email address and home address.
• Appointment and consultation requests, including the time you pick and any notes you add.
• Information about the person receiving care, if you book for a family member.
• For physical therapy patients: health, medical history, physician and insurance information needed to provide and bill for care.
• Limited billing details. Card payments are processed by PCI-compliant payment processors, and we do not store full card numbers.

Information collected automatically

Like most websites, ours records basic technical information when you visit, such as your IP address, browser and device type, the pages you view and how you arrived. Our website platform uses cookies and similar technologies for this and to keep the site working.

3. How We Use Your Information

• To respond to your questions and consultation requests.
• To schedule, provide and coordinate physical therapy and our non-clinical services.
• To send appointment reminders and service updates.
• To bill Medicare, insurers or you, and to process payments.
• To coordinate with your physician or care team when you ask us to or when your care requires it.

4. Health Information and HIPAA

Health information created or received by Opt Physical Therapy Rehabilitation, PLLC in the course of your care is protected health information (PHI). It is used, disclosed and safeguarded in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and New York law. Patients have additional rights over their PHI, including the right to see and get a copy of their records and to ask for corrections.

Please send medical information only through the secure forms or systems we give you for that purpose. Regular email, social media messages and the general contact form are not meant for health information.

5. Text Messages (SMS)

If you give us your phone number, you agree to receive appointment and service-related text messages from us. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

6. How We Share Information

We do not sell or rent personal information. We share it only:

• With our staff and clinicians who need it to provide your care or service.
• With service providers who work for us under confidentiality obligations, such as our website host (Squarespace), our online form provider (Jotform), and our scheduling, records, billing and payment systems. Where these handle PHI, we use HIPAA-compliant services.
• With Medicare and insurance carriers, for billing.
• With your physician or care team, for treatment and coordination.
• With regulators, courts or law enforcement when the law requires it.

7. Cookies and Embedded Content

Our website uses cookies to function and to measure visits. You can block or delete cookies in your browser settings; some parts of the site may not work as well if you do.

Some pages include content from other services, such as embedded videos, podcast players, review widgets and booking forms. Those services may set their own cookies and collect information under their own privacy policies.

8. How Long We Keep Information

We keep information only as long as we need it for the purposes above or as the law requires. Clinical records are kept for the periods required by federal and New York State law.

This website is not directed to children under 13, and we do not knowingly collect personal information from them online. When we provide care to a minor, information is collected from and with the consent of a parent or guardian.

Our website links to other sites and services, such as scheduling tools, billing portals and resources from other organizations. We are not responsible for their privacy practices, so please review their policies before you share information with them.
