Care transition planning supports safer movement between healthcare settings and home, such as hospital discharge, rehabilitation discharge, recovery after illness or surgery, or changing healthcare needs at home.
How can I help a loved one recover safely at home?
Safe recovery may involve rehabilitation, care transition planning, home safety strategies, accessibility considerations, mobility support, and thoughtful environmental planning tailored to the individual’s needs and living environment.

Services, Coverage & Support
How is Opt2Restore different from traditional physical therapy, home care, or a contractor assessment?
Opt2Restore uses a rehabilitation-led lens combining clinical insight, environmental strategy, accessibility thinking, care transitions, and practical implementation guidance to support safer recovery, safer living environments, and long-term independence.
Why choose a rehabilitation-led approach?
A rehabilitation-led approach brings clinical insight into how people move, recover, function, and live within real environments. This perspective helps bridge healthcare needs with practical living considerations, accessibility, recovery, and long-term independence.
What happens after recommendations are made?
Depending on the service selected, support may include practical guidance, planning, next-step recommendations, implementation strategy, coordination support, or referrals designed to help individuals and families move forward with greater clarity and confidence.
Do you only work with older adults?
No. While aging-in-place is a core focus, Opt2Restore also supports adults recovering from illness, injury, surgery, disability, hospitalization, mobility decline, accessibility concerns, and changing healthcare needs at home.
Can you help after surgery, stroke, neurological conditions, injury, disability, hospitalization, or medically complex recovery?
Depending on the situation, services may support individuals recovering from surgery, hospitalization, critical illness, cardiac events, neurological conditions, disability, accessibility challenges, or changing functional needs within home and community environments.
General Questions
How does mobile physical and occupational therapy work?
Mobile PT/OT brings care directly to you—whether at home, work, or another convenient location. Learn more about our in-home physical therapy services and how we personalize treatment plans for each patient.
